Why an Ophthalmologist
for Aesthetics

A lot of people in Los Angeles have a complicated relationship with cosmetic work. They want a result no one can quite name. They don’t want to look done. They’ve seen what bad Botox and overdone filler look like, and they don’t trust easily.

Berg-Feinfield is an ophthalmology practice that does facial aesthetics — not the other way around. The patients who find us are usually looking specifically for an injector with formal training in the anatomy around the eyes, because that’s where most of the work happens.

Most cosmetic injection sites — the forehead, the glabella, the crow’s feet, the under-eyes, the tear troughs, the upper cheeks — sit within a few centimeters of the eye. And our surgical patients considering blepharoplasty benefit from a surgeon who operates on the eyelid and eye daily.

“The result, for patients, is a treatment plan that respects what should still move — a forehead that can still register surprise, a brow that doesn’t pull outward, an under-eye result that doesn’t go blue or lumpy.”

Anatomy

Eye-Adjacent Training

The forehead, glabella, crow’s feet, under-eyes, tear troughs, and upper cheeks are exactly where ophthalmologists train. Most aesthetic injectors learn this same anatomy at a weekend course.

Decades With Botulinum Toxin

Dr. Robert Feinfield has been administering botulinum toxin since 1986 — sixteen years before its FDA approval for cosmetic use.

What People Come In For

Most first visits start with a list. Sometimes the list is short and specific, sometimes it’s a question of where to start. The most common concerns we hear:

  • Excess skin on the upper eyelids that feels heavy or blocks vision — blepharoplasty addresses this surgically
  • Bags or puffiness under the lower lids that haven’t responded to anything
  • Lines from facial expression — the “11s” between the brows, forehead lines, and crow’s feet
  • Volume loss in the cheeks or mid-face that wasn’t there a few years ago
  • Hollowing or shadowing under the eyes that makeup can’t quite cover
  • Thinning lips, or vertical lines around the mouth
  • A previous result somewhere else that you didn’t love and want to address

Before Your Visit

Bring photos of yourself from a few years ago, and any details you have about prior treatments — products, areas, dates. The more we know going in, the better the plan we’ll come out with.

Where Most of the Work Happens

Most cosmetic injection sites sit within a few centimeters of the eye. These are the regions we evaluate and treat most often — plus blepharoplasty for structural eyelid concerns.

01

The “11s” (Glabella)

The two vertical lines between the brows that come from frowning or concentration. One of the most common areas for Botox.

02

Forehead Lines

Horizontal lines from raising the brows. Treated with Botox — dosed conservatively to keep the brow expressive.

03

Crow’s Feet

Lines that fan out from the outer corners of the eyes when smiling. A small, precise Botox treatment that softens without freezing.

04

Under-Eyes & Tear Troughs

Hollowing under the eyes that creates shadow. Treated with Restylane — a more predictable gel structure for this delicate area.

05

Upper & Lower Eyelids

Excess upper lid skin and under-eye bags are addressed surgically with blepharoplasty — not injections. Dr. Feinfield performs both upper and lower procedures.

06

Cheeks & Lower Face

Soft cheek augmentation and lip refinement. Juvéderm is our first choice here — it integrates well in the mid-face and around the mouth.

Blepharoplasty, Botox, and Fillers

Each service has a dedicated detail page with deeper specifics — products, technique, recovery, and costs. Use the cards below to read more.

Botox
Botulinum Toxin · Lines From Movement · 3–4 Months

For lines from facial expression — the “11s” between the brows, forehead lines, crow’s feet, bunny lines, and smaller targeted areas like a lip flip or a chemical brow lift. Conservative dosing: under-treat at the first visit and add at a two-week follow-up if you want more.

Juvéderm & Restylane
Hyaluronic Acid Fillers · Volume & Refinement · 6 Mo–2 Yr

Hyaluronic acid fillers for volume loss, deep lines, and feature refinement. Juvéderm for soft cheek augmentation and lower-face work. Restylane for under-eye hollowing where the gel structure is more predictable. All products are reversible with hyaluronidase.

Combination Treatments

Most patients ask about more than one service. The most common combination is Botox and filler at the same visit. Blepharoplasty patients often add filler or Botox at a separate session for comprehensive facial rejuvenation.

Blepharoplasty + Botox
Most Popular Combination · Staged

Upper blepharoplasty removes structural excess. Botox softens the lines around the eyes and brow that surgery doesn’t address. We typically stage these — surgery first, Botox at a follow-up once the eyelids have healed.

Botox + Filler — Same Visit
Single Appointment · Most Common Injectable Combo

Botox handles the lines from motion. Filler addresses volume loss and lines at rest. The two work on completely different mechanisms and can be placed in the same visit.

Blepharoplasty + Cheek Filler
Staged · Mid-Face Support

Patients considering lower blepharoplasty sometimes pair it with filler in the cheek to support the lower lid from below and address the tear trough simultaneously. We’ll walk through what’s reasonable to combine.

Upper Bleph + Brow Botox
Common · Lid + Brow Together

Upper blepharoplasty and brow Botox address complementary concerns — the surgical removes the excess skin, the Botox maintains ideal brow position and softens forehead lines above.

Your Consultation

A first aesthetic consultation runs 30 to 60 minutes depending on what you’re considering. We talk first — what’s bothering you, what you’ve had done before, what you’re trying to avoid. Dr. Feinfield evaluates your face at rest and in motion and walks through a treatment plan with specific products, areas, and costs before anything is opened.

For patients considering blepharoplasty, Dr. Feinfield performs a complete eyelid examination — assessing skin excess, fat herniation, levator function, and brow position — and will tell you clearly whether you’re a surgical candidate, whether insurance is likely to apply, and what to expect.

“If something you’re asking for isn’t something we’d recommend, we’ll tell you.”

Frequently Asked

The questions we hear most often from patients researching facial aesthetics and blepharoplasty in Los Angeles, Burbank, Sherman Oaks, and Beverly Hills.

Blepharoplasty is surgery — it removes excess skin, fat, or muscle from the eyelids. Botox and filler are injectable. Injectables cannot remove excess eyelid skin or correct structural under-eye bags caused by herniated fat. If the issue is structural — heavy, drooping upper lids or persistent bags under the lower lids — blepharoplasty is the only solution that produces a lasting result. Injectables can complement blepharoplasty but cannot replace it. Dr. Feinfield evaluates whether you need surgery, injectables, or a combination at the consultation. Full blepharoplasty guide →
You don’t need one — most aesthetic injectors aren’t physicians at all. The argument for an ophthalmologist is anatomical: cosmetic injections happen right next to the eye, in the exact region we train on for years. The training matters most for under-eye filler, any Botox above the brow, and blepharoplasty — which is literally eyelid surgery, performed millimeters from the globe.
Yes. The two don’t interact, and it’s a common combination. Botox handles lines from motion, filler handles volume loss and lines at rest.
Botox lasts 3 to 4 months. Filler lasts 6 months to 2 years depending on the product and area. Blepharoplasty results are long-lasting — upper lid results typically last 10–15 years or longer; lower lid fat removal is often permanent.
Upper blepharoplasty may be covered by medical insurance when it is medically necessary — specifically, when excess skin obstructs the superior visual field on visual field testing. Cosmetic upper blepharoplasty and lower blepharoplasty are generally not covered. Other cosmetic procedures (Botox, filler) are always out of pocket. We can evaluate coverage eligibility at the consultation. Full blepharoplasty insurance info →

A lot of our new patients found us after a result they didn’t love. Most bad Botox fades on its own within three months; most HA filler can be dissolved with hyaluronidase if you don’t want to wait it out. Bring photos and details about what was placed, if you have them.
